What information is required to complete an online audit?

To complete your online audit, you will need the following information and documents:

  • Breakdown of actual payroll by classification.
  • Annual gross income for all officers, along with the appropriate classification code for each officer. (See the question below for more information about what is included in annual gross income).
  • State quarterly SUTA reports for the coverage period (all pages must be uploaded through the Member Portal).
  • Internal payroll reports for the audit period (must be uploaded through the Member Portal).
  • All 1099 forms issued, along with the 1096 form and certificates of insurance for all subcontractors (must be uploaded through the Member Portal).
  • Any changes that occurred during the audit period, including changes in ownership, business operations, partners, executive officers, locations, entity type, or contact information.

What types of payroll are included in total gross salary/wages?

Total gross salary and wages include all forms of payroll, including salary, wages, overtime, commissions, draws, bonuses, 401(k) contributions, cafeteria plans, 403(b) contributions, holiday pay, vacation pay, sick pay, piecework, jury duty, and other compensation. Include payroll for all employees who perform the same type of work and report the total under the classification that best describes the work performed.
